Schedule (Section 17(b))

Part 1: Particulars Affecting Entitlement to Payment or the Rate thereof for the Purpose of Section 17(b)

In this Schedule, "the basic amount" – the amount referred to in paragraph (3) of the definition "the basic amount" in the National Insurance Law;

1.§

The existence or non-existence of an account balance of the beneficiary at a financial institution, as defined in the Agreements on Financial Assets Law, 5766-2006, whether in his or her sole ownership or in joint ownership with another, or a change that has occurred therein, if such change exceeds 9% of the basic amount in respect of a single individual, or 13% of the basic amount in respect of a person who has with him or her a child or spouse.

2.§

The existence or non-existence of a right in real property of the beneficiary, or a change that has occurred in such right.

3.§

The amount of income from employment in which the beneficiary is employed, or a change that has occurred therein, if such change exceeds 9% of the basic amount in respect of a single individual, or 13% of the basic amount in respect of a person who has with him or her a child or spouse.

4.§

Income from other sources of the beneficiary or a change that has occurred therein, if such change exceeds 9% of the basic amount in respect of a single individual, or 13% of the basic amount in respect of a person who has with him or her a child or spouse.

5.§

The residential address of the beneficiary in Israel, or a change in such address, or a move to another residential address outside Israel.

6.§

Whether the beneficiary holds foreign citizenship, or a change in respect of such citizenship.

7.§

The dates of the beneficiary's entry into and departure from Israel, and the period of his or her stay outside Israel.

8.§

Residing under the same roof, the commencement or cessation of such residence, with any of the following:

(1)a person who has married the beneficiary, including a person who has married under the law of a foreign state;
(2)a person who is a parent of one of the beneficiary's children;
(3)a person with whom the beneficiary holds a joint bank account or with whom the beneficiary holds a joint right in real property or in a vehicle;
(4)a person with whom the beneficiary has a financial agreement or other agreement concerning a sharing of income or expenses.
9.§

The receipt by the beneficiary of funds directly from the debtor or from a person acting on the debtor's behalf for the purpose of Section 12, or the non-receipt of such funds.

10.§

The existence or non-existence of a maintenance judgment in favour of the beneficiary that is later than the judgment submitted by the beneficiary to the Institute.
