Schedule (Section 5)

(1)Support services for independent and autonomous living and therapeutic-rehabilitative housing services;
(2)daily activity services, including rehabilitation services by means of adapted employment and occupation;
(3)health services provided within the framework of therapeutic-rehabilitative housing services or within the framework of daily activity services for the purpose of integration in such a framework;
(4)transportation services for daily activity as referred to in paragraph (2), including escort during transportation;
(5)transition programmes from the education system to the welfare services system;
(6)support services for the purpose of adapted integration in civic service as defined in the Civic Service Law, 5777-2017, or in the Israel Defence Forces;
(7)leisure services;
(8)personal escort or personal assistance services, including for the support of independent and autonomous life in the community;
(9)counselling, guidance and additional services for the family of an eligible person, including short breaks and services in times of crisis;
(10)counselling, guidance and additional services for a parent who is himself a person with a disability;
(11)services for the enhancement of the integration of an eligible person in society, including services in the following matters: information, counselling and support services, personal accessibility services, services for the acquisition of functional skills in various subjects, guidance and treatment services, including in the socio-sexual domain, and guidance and coaching services in the technological domain, including assistance in the acquisition of technological means.
